ABSTRACT

Paederus dermatitis is a characteristic irritant contact dermatitis caused by paederin which is a chemical substance released by insects of the genus Paederus. Erythematous vesiculobullous eruption with a burning sensation occur within 24 hours after contact with Paederus. This dermatitis is frequently seen in regions with hot, tropical climates such as Africa and the Middle East. There have been a few reports of Paederus dermatitis in Korea. We recently had a patient with Paederus dermatitis who traveled to Kenya. We would like to highlight an awareness of this peculiar dermatitis due to the increasing numbers of travelers visiting tropical countries.

ABSTRACT

Black hairy tongue occurs on the dorsum of the tongue, and is characterised by a black, yellowish, or brown plaque with hairlike structures which result from benign hyperkeratosis of the filiform papillae. It may be associated with several conditions such as smoking, oral antibiotics or candida infection. However, the definite etiology is unknown. We report a case of black hairy tongue without any associated condition.

ABSTRACT

Congenital smooth muscle hamartomas (CSMHs) usually appear at birth as skin-colored or slightly hyperpigmented patches or plaques on the trunk or extremities and are often covered by and increased amount of vellus hair. Histopathologically, it represents a proliferation of smooth muscle bundles within the dermis(1,2). We herein report on three cases of localized CSMHs; Case 1 was a 2-month-old boy who presented with a skin-colored, 6x5cm-sized plaque with folds, hypertrichosis and pseudo- Darier's sign on the face, an unusual site. But there was no hyperpigmentation. Case 2 and 3 were both 1-month-old boys who presented with typical hyperpigmented, hairy plaques on the lateral aspect of the left thigh.

ABSTRACT

BACKGROUND: More than 10 years have passed since the second multicenter study on standard patch test were carried out in Korea (1986-1993) and an update is needed. OBJECTIVE: The purpose of this study was to analyze the type, frequency and changes of the common contact allergens in Korea. METHODS: Standard patch tests were performed using Finn chamber on Scanpor tape on in-patients and out-patients with contact dermatitis from January 2000 to July 2006. Epidemiologic findings of patients and the results of the patch tests were analyzed. RESULTS: A total of 528 patch tested cases (male 135, female 393) were compiled and analyzed. The highest age distribution was 5th decade in males and 3rd decade in females. Patients' employment background showed that housewives and office-workers were the most frequently expected jobs consisting 32.4% and 15.2% respectively. The face, including eyelid and lip, was the most frequently affected site comprising 61.2%. The highest sensitization rates were found with : nickel (31.1%), neomycin (9.3%), cobalt (6.4%) and potassium (6.3%). The lowest positivity included : Me-Cl-Isothiazolinone (0.4%), 7-mercaptobenzothiazole (0.8%), black rubber mix (0.8%) and wool alcohol (0.8%). While nickel and neomycin showed increased reacitivities, formaldehyde, fragrance mix and colophony showed a decreased tendency when compared with those of other previous studies. CONCLUSION: These results provide the basis for the presumption of the route of allergen exposure and prevention of contact dermatitis in Korea.

ABSTRACT

We report here on a case of secondary cutaneous diffuse large B-cell lymphoma (DLBCL) that occurred in a 67-year-old man who had a 2-month history of nodular growing masses on the left cheek, plus palpable lymph nodes on the left cervical area. The histopathological findings showed a diffuse infiltration of large atypical lymphocytes with nuclear atypia throughout the entire dermis. These showed positive CD20, bcl-2 and the post-germinal center marker, MUM-1. According to the WHO (World Health Organization) classification, this lymphoma is considered to be diffuse large B-cell lymphoma of the post-germinal center (GC) B-cell type with a secondary cutaneous manifestation. We treated the patient with systemic chemotherapy (CHOP) and anti-CD20 monoclonal antibodies. During the course of treatment, new skin lesions developed on his neck, so we changed the regimen to cytosin- arabinoside and cisplatin. But he died of pneumonia after the third cycle.

ABSTRACT

Chondroid syringoma is an uncommon, benign neoplasm of sweat gland origin. It clinically presents as a slowly-growing intradermal or subcutaneous nodule, and is usually observed on the head and neck of middle-aged to elderly people. Histologically, two types of chondroid syringoma can be recognized: one common type has tubular and cystic lumina and the other extremely rare type has small tubular lumina. We herein report a rare case of chondroid syringoma with small tubular lumina.

ABSTRACT

Intramuscular lipoma, also referred to as an infiltrating lipoma, is a rare, benign uncapsulated tumor that originates between skeletal muscle bundles and infiltrates through the intramuscular septa. These lipomas are generally found on the upper or lower limbs, but rarely occur on the face. We report a case of intramuscular lipoma of the chin, which has not ever been reported in Korea. Histopathological findings showed mature adipocytes infiltrating muscle in a diffuse manner. The lesion was excised completely and has shown no evidence of recurrence.

ABSTRACT

PURPOSE: We performed the present study to investigate whether osteotropic hormomes play roles on the nitric oxide (NO) production in culture of ROS17/2.8 osteoblastic cells. MATERIALS AND METHODS: The osteoblastic cell line ROS17/2.8 cells were cultured in F12 medium supplemented with 5% fetal bovine serum (FBS) at 37 degrees C in a humidified atmosphere of 5% CO2 in air. ROS17/2.8 cells were plated in 96-well plates at a density of 2-3x10 (3) cells/well and grown to confluence. Then the cells were pretreated with osteotropic hormones (parathyroid hormone (PTH) 20-500 ng/mL, 1, 25-dihydroxycholecalciferol (1, 25[OH]2D3) 1-100 nM; prostaglandin E2 (PGE2) 20-500 ng/mL) in the medium supplemented with 0.4% FBS for 72 hours and the cells were treated with cytokines (TNFalpha and IFNgamma) in phenol red-free F12 medium for an additional 48 hours. NO synthesis was assessed by measuring the nitrite anion concentration, the reaction product of NO, in the cell culture medium using Griess reagent. RESULTS: PTH and 1, 25[OH]2D3 pretreatment induced a significant increase in NO production in the presence of TNFalpha and IFNgamma. PGE2 slightly induced NO production compared to the control group. But, PGE2 pretreatment did not affect in NO production in the presence of TNFalpha and IFNgamma. CONCLUSIONS: These results suggest that the actions of osteotropic hormones in bone metabolism may be partially mediated by NO in the presence of cytokines.

ABSTRACT

This study was performed to define the cytotoxicity and the anti-inflammatory action of Pulsatilla koreana extracts. To analyze cytotoxic effects, gingival and periodontal ligament fibroblasts were used, and anti-inflammatory actions related to reduction of IL-1beta and PGE2 production were performed in vitro, for the suggestion of efficacy and safety on periodontal therapeutic use of Pulsatilla koreana extracts. We extracted ethylacetate and butylalcohol from well-dried and ground Pulsatilla koreana throughout multiple processing, then used different concentration solution(0.1 %, 0.2 %, 0.4 %, 0.01 %, 0.02 %, 0.04 %, 1 %, 2 %) of ethylacetate and butylalcohol extracts to examine cytotoxic effects and anti-inflammatory actions Cytotoxic effects were examined by ELISA reader using MTT(Methyl Thiazol-2-YL-2, 5-diphenyl Tetrazolium bromide)solution following culture of human gingival and periodontal ligament fibroblasts. Synthesis of IL-1beta was examined by IL-1beta enzyme-immunoassay(EIA)system after separation and culture of monocyte, and PGE2 was examined by PGE2 EIA system after culture of gingival fibroblasts. The results were as follows: 1. In the MTT test of gingival fibroblasts, the change of optical density was decreased significantly at 2 % of butylalcohol extracts and 0.04 %, 0.1 %, 0.2 %, 0.4 %, 1 %, 2 % of ethylacetate extracts.(p<0.05) 2. In the MTT test of periodontal ligament cells, the change of optical density were not differ significantly. but butylalcohol and ethylacetate extracts except from butylalcohol 0.01 % showed high cell cytotoxity. 3. Both ethylacetate and butylalcohol extracts from Pulsatilla koreana inhibited the synthesis of IL-1beta, and inhibition effect of ethylacetate extracts were higher than butylalcohol extracts. 4. Both ethylacetate and butylalcohol extracts from Pulsatilla koreana inhibited the synthesis of PGE2, and ethylacetate extracts were higher than butylalcohol extracts. In conclusion, ethylacetate and butylalcohol extracts from Pulsatilla koreana showed little cell cytotoxity for gingival and periodontal ligament fibroblasts, and the inhibition of IL-1beta and PGE2 synthesis, therefore it is considered that these extracts can be developed as the therapeutics of the periodontal disease.

ABSTRACT

For reconstruction of the bony defect, various artificial substitutes were developed. Among them, there has been a study of calcium phosphate coated bone substitutes for increasing attachment of osteoblasts in vivo. The purpose of this study was to evaluate the effects of serum and platelet-rich plasma (PRP) on calcium phosphate coated culture plate for the initial attachment, proliferation and activity of osteoblasts. After sampling the blood from white rats and concentrating by centrifugation, the amount of attachment of PDGF-BB and TGF-beta on the calcium phosphate coated culture plate was measured. Cultured HOS and ROS 17/2.8 cell was measured on attachment level and proliferation rate of osteoblasts. Alkaline phosphatase activity of HOS and ROS 17/2.8 cell was measured for studying on the activating rate of osteoblast. 1. Counting the amount of platelets of seperated plasma and PRP, the average number of platelets was 177,003 cell/microliter in plasma, and 1,656,062 cell/microliter in PRP, which was about 9 times as high as in plasma. 2. Amount of PDGF-BB deposited at calcium phosphate coated plate had increased by the total amount of plasma and PRP on the culture plate, whereas TGF-betahad been deposited on the plate only when treated by 50microliter of PRP(p<0.01). 3. After plating serum and PRP for 3 hours, we attached with HOS and ROS17/2.8 cell for 1 hour and 4 hours. There were no significant difference of the attachment between serum and control group, whereas there were significantly difference of the attachment between depositioning of PRP and control group. 4. After attaching plasma and PRP for 3 hours, cell number has much increased when HOS and ROS17/2.8 cell had been cultured for 48 hours(p<0.05). 5. After attaching plasma and PRP for 3 hours, concentration of alkaline-phosphatase has increased when HOS and ROS17/2.8 cell had been cultured for 48 hours(p<0.01). These results suggested that PRP affected on initial cell attachment rather than proliferation and activation of osteoblasts at calcium phosphate coated plate.

ABSTRACT

Natriuretic peptides comprise a family of three structurally related peptides; atrial natriuretic peptide (ANP), brain natriuretic peptide (BNP), and C-type natriuretic peptide (CNP). The present study was performed to investigate the effect of ANP on the proliferation and activity of ROS17/2.8 and HOS cells which are well-characterized osteoblastic cell lines. ANP dose-dependently decreased the number of ROS17/2.8 and HOS cells after 48-hour treatment. ANP generally increased the alkaline phosphatase activity of ROS17/2.8 and HOS cells after 48 hr treatment, regardless of the fact that basal activity of alkaline phosphatase was much lower in HOS cells compared to that of ROS17/1.8 cells. ANP increased the NBT reduction by ROS17/2.8 and HOS cells. ANP showed the variable but no significant effect on the nitric oxide production by ROS17/2.8 and HOS cells. ROS17/2.8 and HOS cells produced and secreted gelatinase into culture medium, and this enzyme was thought to be the gelatinase A type with the molecular weight determination. The gelatinase activity produced by ROS17/2.8 cells was increased by the treatment of ANP. However, the enzyme activity was not affected by ANP treatment in the HOS cell culture. In summary, ANP decreased the proliferation and increased the alkaline phosphatase activity and NBT reduction of osteoblasts. These results indicate that ANP is one of the important regulators of bone metabolism.

Gingivitis and periodontitis are infectious diseases in that microorganisms are the primary extrinsic cause of the diseases. the occurrence of gingivitis has been associated clearly with the presence of microorganisms at the disease site, and the histologic nature of the tissue involved is indicative of an inflammatory response induced by microorganisms. additional evidence for the microbial etiology of periodontal disease is that numerous antimicrobial agents are effective in reducing plaque accumulation and periodontal diseases. the purpose of this article is to analyze the antimicrobial effects of Pulsatilla koreana. Well-dried Pulsatilla koreana purchased from herbs distributor was ground and extracted into methanol(MeOH), ethylacetate(EtoAc), chlorform(CHCl3) and Butyl alcohol(BuOH). we have then applied each solution to the bacteria samples(Bacteroides forsythus, Streptococcus mutans, Streptococcus sanguis, Porphylomonas gingivalis, Actinobacillus actinomycetemcomitans, Eikenella corrodens, Prevotella intermedia, Actinomyces viscosus, Prevotella nigrescens, Rothia dentocariosa, Fusobacterium nucleatum, Pseudomonas aeruginosa, Staphylococcus aureus) collected from several organizations. To conduct susceptibility test(Kirby-Bauer method), plate contained each periodontopathic bacteria is spread extracted into methanol(MeOH), ethylacetate(EtoAc), chlorform(CHCl3) and Butyl alcohol(BuOH) and to measure the minimum inhibition concentration(MIC) of the bacteria against the solutions to ultimately determine antimicrobial effects of the solutions, insert bacteria sample into 20microliter/ml, 10microliter/ml, 5microliter/ml, 2.5microliter/ml of each solution and control group(not contained solution) 1. Solution extracted into methanol did not show clear zone against all bacteria samples. Only P. nigrescens, S. mutans and S. sanguis in soluton extracted into ethylacetate, S. mutans and S. anguis in solutions extracted into chlorform and Butyl alcohol showed clear zone against all bacteria samples. Solution extracted into Butyl alcohol showed clear zone against 13 types of bacteria, excluding P. gingivalis. 2. In Solution extracted into methanol, the bacteria samples grew in the highest concentrated plate, showing minimal variation from control group. 3. In Solution extracted into Butyl alcohol, S. aureus, P. intermedia, E. corrodens, A. actinomycetemcomitans, B. forsythus, P. gingivalis et al. showed decreased growth in the highest concentrated plate. P. auruginosa, R. dentocariosa, A. viscosus, P. nigrescens, S. mutans et al. showed decreased growth at MIC 20microliter/ml and S. sanguis showed decreased growth at MIC 10microliter/ml. 4. By analyzing the MIC level through considering the results from Kirby-Bauer method, Solution extracted into methanol did not reveal any antimicrobial effects and Solution extracted into Butyl alcohol showed the highest antimicrobial effects In conclusion, it can be used the extracts of Pulsatilla koreana as wide spectrum antimicrobial agent.

Fetal teratomas rarely complicate pregnancy,having an incidance of only 20,000:1 to 40,000:1 of live births. Overthere, cervical teratomas are rare and accounts for only 5.5% of all neonatal teratomas. We have experienced a large cervical immature teratoma and present this case with a brief review of literatures.

ABSTRACT

BACKGROUND: Juvenile myoclonic epilepsy(JME) is the most common idiopathic generalized epileptic syndrome which occurs mostly in the second decade of life. It is still frequently unrecognized and misdiagnosed. JME responds well to valproate, but is hardly controlled by other commonly used antiepileptic drug such as carbamazepine or phenytoin. We intended to Investigate the clinical symptoms and EEG findings of JME to support the diagnosis and treatment of JME. SUBJECTS AND METHODS: We retrospectively reviewed the medical record of 18 childrens with JME, from March 1991 to February 1997 We have analysed the clinical symptoms, seizure type, EEG findings including photosensitivity, and the effect of antiepileptic drug. RESULTS: 1) Seven patients were boys and eleven patients were girls(M : F=1 : 1.6). The seizure onset between 5 and 7 years of age were noted in 5 cases, that between 8 and 10 years in 5 cases, and in 8 cases seizures began after 11 years of age 2) Generalized convulsive seizures were noted in 16 cases(GTC 13, GC 2, GCTC 1), myoclonic seizures in 18 cases, absence seizures in 5 cases, and photoconvulsive seizures during video game were associated in 3 cases. 3) Epileptiform discharges at the diagnosis of JME were noted 12 cases(66.7%), and no epileptiform discharges were seen in 6 cases(33.3%). The background activities were normal in all cases. In all 12 cases that showed abnormal epileptiform discharge, generalized spike and wave discharges were obtained, and photosensitivies noted in 4 cases. 4) Follow up EEG obtained after treatment, showed normal EEG findings in 16 cases, and abnormal EEG findings were obtained in 2 cases. 5) Sixteen cases responded well to valproate monotherapy, and 2 other cases responded to valproate and ethosuximide combined therapy. In one case who received vigabatrin, the seizure was aggravated. 6) In 5 cases who discontinued antiepicoptic drug medication after 3 year seizure free duration, 3(60%) cases relapsed within 1 year, and 2(40%) cases remained seizure free for over 1 year. CONCLUSION: Juvenile myoclonic epilepsy is an epileptic syndrome with generalized convulsive seizure, myoclonic seizure, and absence seizure, and EEG findings showed generalized spike and wave discharge in 66.7%, and photosensitivity in 22.2%. JME responded well to valproate monotherapy or valproate and ethosuximide combined the rape, but not responded to other antiepileptic drugs.

Effects of several cytokines(IL -1beta, TNFalpha, and IFNgamma) have been examined on fetal rat osteoblast-like cells. To investigate whether cytokines play direct causal roles in production of lysosomal enzyme, fetal rat osteoblast-like cells were treated with IL-1beta, TNFalpha, and IFNgamma, respectively or combined. And acid phosphatase was determined by biochemical method. Alkaline phosphatase was assayed to determine the effects of IL-1beta, TNFalpha, and IFNgamma on the expression of this enzyme. And also experiment of calcified nodule formation was performed to assess the effects of cytokines on the bone-forming activity of osteoblast-like cells in vitro. Acid phosphatase activity was significantly increased by the addition of IL-1beta and TNFalpha, whereas decreased by IFNgamma, However, no significant changes in alkaline phosphatase activity was observed when the osteoblast-like cells were treated with IL-1beta and TNFalpha. Interestingly, IFNalpha showed stimulatory effect on alkaline phosphatase activity. The number of calcified nodules was decreased by treatment of cultures with 1ng/ml IL -lbeta, 20ng/ml TNFalpha, and 500 u/ml IFNgamma continuously for 21 days, while considerable number of calcified nodules were formed in control group of osteoblast-like cell in culture for 21 days. These results seem to suggest that cytokines may play crucial roles in bone remodeling through the direct action on the osteoblast-like cell.

ABSTRACT

BACKGROUND: To pass through the microcirculation, erythrocytes need to undergo considerable deformation. We performed an experiment to investigate the erythrocyte deformability in healthy adults and in patients with uncontrolled hypertension and controlled hypertension. METHODS: 5ml of whole blood is anticoagulated with 2mg ethylenediaminotetra-acetic acid(EDTA). 1ml graduated plastic syringe, without its plunger, is filled with anticoagulated whole blood to the 1ml mark, And the syringe is put into the assembled membrane holder. Under standard condition, whole blood is passed through a 5micro polycarbonate membrane filter under negative pressure of 20cm H2O. The studies are performed at roon temperature and within two hours of venipuncture. RESULTS: Erythrocyte deformability was observed in 91 normal healthy adults and 25 patients with uncontrolled hypertension and 18 patients with controlled hypertension. Following results were obtained. 1) In male control age group I, II, III, the erythrocyte deformability indices were 0.776+/-0.125, 0.792+/-0.189, and 0.714+/-0.172 respectively and in female the indices were 0.880+/-0.138, 0.865+/-0.182 and 0.796+/-0.170 accordingly. 2) THe RBC deformability indices in female control troup were significantly higher than the male group in statistics(p0.05). CONCLUSION: This study reveals that the erythrocyte deformability in patients with essential hypertension is significantly lower than the healthy adults.
